I understand and accept the conditions of participating in any Chaka-Runa Program. I declare I have not been forced or persuaded to participate by the organizers or other people. The decision to be part of the Program is only mine and is made on my own free will. I am also fully aware of the effects, potential benefits and risks involved when participating in this experience.

 

I understand and accept that the Chaka-Runa services and/or products are offered "as is" and "as available". I agree that the use of all or part of the services and/or products is done at my own risk. Any action or lack of action carried out by me, regarding practices, guidelines, advice and recommendations provided by Chaka-Runa, is only due to my decision and responsibility and I understand and accept that it is not the responsibility of Chaka-Runa.

 

I understand and accept that I will have to answer any questions asked by the organisers truthfully during the screening process, and complete the application forms and questionnaires honestly. I commit to provide any, and all, relevant information about my medical, mental, emotional and physical health to ensure my safety throughout the Program. 

 

I understand that the information provided to Chaka-Runa is strictly confidential, and will not be shared nor used without my consent for any other purpose. I understand that I am free to ask questions throughout the Program and that, should I change my mind, I may cancel my participation and leave the Program at my own cost and responsibility.

 

I acknowledge and confirm that I will not leave the venue of the Program without the consent of the organizers, and I commit to following the instructions at all times. I understand and agree that in case of infringing the exclusion criteria, rules and guidelines, I can be separated from the Program at any time at the discretion of the facilitators, and be asked to leave the Program and the venue at my own cost and responsibility.

 

Chaka-Runa reserves the right to refuse and deny to provide any service for any reason before or after the Program starts. You are responsible for good behavior during the Program and respecting others’ right to a safe, peaceful, healing environment.

 

We request that all participants start their journey with commitment, respect, an open mind, and an open heart. Focus should be centered at all times around safety, healing, self-respect, respect for others, and respect for the sacred medicines. Thus, the Program environment is substance free, and participants who bring prohibited substances (alcohol, medications, recreational drugs and other plant medicines or herbal remedies) will not be able to participate.

 

The following behaviors will not be accepted: invading personal space, swearing, judgement, insults, bullying, violent and disrespectful verbal or body language or any type of discrimination (race, religion, gender, color, culture, language). In case of infringement the participant will be asked to leave the venue temporarily or permanently, to ensure the safety and comfort of the other participants.

 

I expressly understand and agree to submit to the jurisdiction of the courts of the country, state, province or territory, determined solely by Chaka-Runa to resolve any legal matter arising from this agreement or related to my use of Chaka-Runa products and services.

 

I CERTIFY THAT I HAVE READ THIS DOCUMENT AND I FULLY UNDERSTAND AND ACCEPT ITS CONTENT AND I SIGN IT ON MY OWN FREE WILL IN APPROVAL.
